Patient Portal Tools
- Profile- update preferred name, mobile number and mobile carrier.
- Health History- view and update allergies, medication, and personal health history.
- Appointments- schedule routine and same day medical appointments, view and cancel existing appointments, and complete appointment specific questionnaires.
- Referrals- view and print referrals.
- Handouts- view and print informational handouts from your SHAC provider.
- Messages- review messages sent by providers and staff from SHAC.
- Letters- view and print letters from your SHAC provider.
- Forms- complete required consent, insurance, and other forms prior to your scheduled appointment.
- Insurance card- scan and upload image of your current insurance card.
- Account Summary- view charges and account information from SHAC and print billing statements.
- Immunization History- review and print immunization records for immunizations administered at SHAC.
Patient Portal Log-In
- Go to the MyChart.
- Log-in to the secure browser with your Odin username and password.
- Confirm your date of birth and click “Proceed", which will bring you to the MyChart® homepage.
- Make your selection from the menu on the left.
If you experience technical difficulties with MyChart®, please email firstname.lastname@example.org. Your questions will be answered within 24 hours on weekdays and 72 hours on the weekends.
